ABOUT THE ROLE
We are hiring a support worker to support the team in Wembley to provide housing related support to vulnerable adults and those from various and complex backgrounds, to support them with independent living, providing a low-level preventative service, with support to improve their skills, resilience, and pathways to independence.
You will provide direct communal support to participants by managing a caseload with key working responsibilities. You will work alongside community partners and other key stakeholders to ensure needs, objectives, and requirements of the service are met as a team and individually. You will further be involved in providing effective outreach support to vulnerable adults to assist with tenancy sustainment, independent living, and promoting self-reliance.
Rota: Monday to Friday ranging from 08:00 - 16:00 or 10:00 - 18:00
ABOUT THE SERVICE
Synergy, Brent is a Housing Related Support (HRS) Service for a range of vulnerable people who have been or are at risk of homelessness. The age eligibility criteria for the service is adults between the ages of 18 to 60, who are identified to have low/medium support needs. Some of the needs we support are:

ABOUT YOU
We’re looking for an individual who is driven to provide high quality care and support to others, someone who thrives on working in a team, but can also make individual judgements in a proactive manner. You will be confident in working in the given setting and can be compassionate, supportive, and empowering to others with the ability to form effective, positive, and motivational relationships! We work alongside various partners and stakeholders so we are looking for a Support Worker who can adapt and work well alongside other teams.
OVERVIEW OF KEY RESPONSIBILITIES
Homeless Prevention Officers, Local Landlords, Housing Benefit Team, Allocations Department, Temporary Accommodation, Housing Associations, Hostels, Supported Housing Schemes, Rehab Facilities.
Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position.
