ABOUT THE ROLE
As a Personal Health and Wellbeing Practitioner, you will provide a person centred practical and emotional support service with therapeutic interventions which is responsive, and caring. You will create an enabling environment and service for the given residents/participants in line with their agreed care plans and risk assessments, offering practical based care, ensuring that the service offered promotes the rights of the individual which preserves dignity, choice, privacy, independence, fulfilment and their personal goals.
Shift Pattern: Rota will include days, nights, evenings, weekends and bank holiday working which can vary between 07:00 - 20:30, 19:30 - 07:30. Please note we have a priority for workers who would prefer working nights although other shifts may also be available.
We will be having a recruitment day event in November which will consist of an introduction to the service and a group interview. we will be conducting initial screening in the week beginning 28th of October.
ABOUT THE SERVICE
Aspinden Care Home (ACH) is a CQC registered care home based in Bermondsey, South East London. It has a national remit and cares for those with complex needs and/or those with long term alcohol addiction. ACH offers residents the chance to gain stability in their lives and improve their physical health and mental well-being.
